Care Navigation Team

A team of peer support specialists

After an initial crisis response, the Care Navigation Unit provides long-term support and stability for community members.  

What is Care Navigation?

The Care Navigation team connects with community members after their initial contact with a first response team. Care Navigation may also connect following a crisis for a community member.  

The team will work to identify specific needs and match the person with available community services. This team will also better connect the various City services needed to support the person. This effort aims to ensure no one falls between the cracks.  

Pilot Scope

The Care Navigation Team will initially focus on housing-compromised and unhoused people. The team will have capacity for 30 active cases at a given time.

  • High Need (lower case turnover): seven-nine cases (30 percent)
  • Medium Need: seven-nine cases (30 percent)
  • Light Touch (higher case turnover): seven-nine cases (30 percent)
  • Other/Variable: two-three cases (10 percent) 

The team will operate within standard business hours: Monday-Friday 9 a.m. to 5 p.m.
