The City’s Annual Wellness Assessment (AWA) is now open and runs until July 31, 2025. The program is designed to support and improve employee health and wellness. The AWA program consists of three components: Biometric Screening, Health Risk Assessment, and an Additional Wellness Activity. Completing the AWA requirements will make you eligible for a health insurance premium discount and provide valuable health insights. If you have a covered spouse, both of you must complete all required components to qualify for the discount starting in January 2026.
To get started, employees, retirees, and covered spouses must individually create a WebMD ONE account or download or download the Wellness at Your Side app with connection code “Raleigh”. Additional information is provided on this flyer.
Biometric Screening
To complete the biometric screening, you have two options:
- City-Sponsored Onsite Screenings: The City’s Human Resources Department has with WakeMed to offer on-site biometric screenings from April to late July 2025. These screenings will be held at various times and locations to accommodate as many participants as possible. To find screening locations, times, and dates or to register for an onsite biometric screening, visit: WakeMed. The onsite biometrics data will be collected and uploaded to the WebMD ONE portal by the screening vendor. Individuals who are pregnant or on leave may qualify for an exemption. Please reach out to Wellness for more information.
- Primary Care Provider: If you completed a physical with your healthcare provider between Aug. 1, 2024 and July 31, 2025, download the 2025 Biometric Screening Form and have your provider complete it. Once the form is completed with the required metrics and signatures, you may upload it here. Alternatively, the form may be faxed or mailed to the number or address listed at the bottom of the form.
If you are pregnant or on leave, you can contact wellness@raleighnc.gov prior to the July 31, 2025 deadline for a potential exemption to the biometric screen. You will still need to complete the health risk assessment and a wellness activity by July 31, 2025.
Health Risk Assessment (HRA)
This is a questionnaire that identifies potential health risks based on your lifestyle, medical history, and environmental factors. Completing a health risk assessment is important for gaining insights into one's personal health status, identifying potential risk factors, and proactively addressing lifestyle choices that may impact well-being. To complete this requirement, you may access the assessment in your WebMD ONE portal. It is accessible on your homepage or by clicking on the menu option.
Additional Wellness Activity Participation
You can complete one of the following:
- City-sponsored virtual or onsite event (Register)
- Age-appropriate preventative screening (e.g., Dental cleaning, vaccinations, mammogram, prostate screening, cervical cancer screening, colorectal screening or vascular screenings completed between Aug. 1, 2024-July 31, 2025).
- Counseling or coaching (e.g., Nutrition counseling, health coaching or mental health counseling)
- WebMD ONE portal activity (e.g., Log 600 minutes of exercise, 150,000 steps, or track sleep for 21 days, complete a wellness challenge, or complete a daily habits plan)
Activity participation may be reported on your “Rewards” page in the WebMD ONE portal. If you have any questions about whether an activity counts for this requirement, contact Human Resources at 919-996-3315 or email wellness@raleighnc.gov.
Verify Completion of AWA
To verify completion of all AWA components, you may click on your “Rewards” page or tile in the WebMD ONE Portal. When all available rewards are completed, the page will look like the image below. Additionally, you will receive an email from WebMD verifying your completion.
Spouses covered under the City’s medical insurance must also complete all AWA requirements.
AWA Communication
To communicate with employees, retirees, and covered spouses about the 2025 Annual Wellness Assessment (AWA) program, the following outreach efforts are planned:
- Mailing to all employee, retiree, and covered spouses home addresses - Look out for an information packet from the City’s Human Resources Department with step-by-step instructions on how to complete the AWA requirements. Packets will be mailed in early spring.
- Automated text messages and emails from the Human Resources Department - Reminders will be sent via text and emails to all employees, retirees, and covered spouses enrolled in the City’s health plan, encouraging them to complete the AWA process to secure savings on health care premiums.
- Portal reminders from WebMD ONE - These reminders will be sent monthly to WebMD ONE portal members who have not yet engaged with the platform to participate in the AWA.
